Obstetrics and Gynecology Claim Form is the medical insurance plan for unborn ( prenatal ) babies. This claims can be submitted as early as 14 weeks of pregnancy and not more than 35 weeks of pregnancy ( which may vary for every insurance provider )

These claims form only valid for 30days from the mothers’ last visit to next follow up visit within pregnancy period of 14weeks to 35 weeks.

If the claims form is considered as expired due to late submission from last visit’s date , a customer has to bare to pay for another claim for attending consultant to prepare it.

A mother or an agent have to be assured on mother last follow up visit date to ensure the report is still valid in order to process the claim submission.

As example , Jane last follow up visit was 12th Aug on her 18 weeks pregnancy. Jane next follow will be on 13th Sept hence the claim should be submitted to us on first ( 1st) week after follow up to ensure attending consultant able to prepare the report within our timeline.

If an agent/mother send to us later from the follow up date, we will advise to process based on next date of follow up visit to avoid the claims will be expired.
